To Make an Appointment

Please call 707-262-5076 Monday - Friday, 8am to 5pm to make an appointment. Appointments are preferred. "Walk-in" visits will be accommodated as the schedule allows. If you need to cancel an appointment, please call at least 4-5 hours prior to your scheduled appointment time.

Please bring your Photo ID and Insurance Card to EVERY VISIT!
